On another forum that I frequent, it seems that the moderator logs are not reliably logging all that they should on person's activity in particular appears to be going unrecorded, but it seems that there are other omissions.

The most obvious answer is that he has been manipulating the logs, but that does not seem entirely convincing for a couple of reasons.

Are phpBB logs generally a bit flaky? Or is there some kind of stealth mode that you can use to moderate off the record?

Dunno. It logs some things, fails to log others. Some of it is centred around whether there's been an 'action' by another user before the moderators action: If here hasn't been then the log doesn't show the moderation ISTR. And there's some issues around self-moderation.
