I nearly trod on one of those in the Maldives when an utter imbecile on a kite board decided to go haring through the narrow swimmers-only channel through the reef. As I was snorkelling with my young son at the time and therefore had my face in the water, I didn't know he was even coming until he flashed right across in front of us, no more than a couple of feet from our faces.
I hope he still considers himself lucky that he disappeared from the beach by the time I'd got us both out of the water.
